Please know that any over-the-counter and prescribed medications are not allowed at school unless we have a written authorization from your doctor.
- If your child needs medication at school please print the forms below and take them to your Child’s Physician to be filled out. Once your Physician has completed the forms, you can deliver the completed paperwork and medication to the health office. (The Dr. may also fax these forms directly to 760-385-4254)
REMINDER: Children are not allowed to transport medication to or from school per board policy.

If your child has a life threatening or other food, bee or other allergy, print the forms below in addition to the two above for your physician to complete and return these to the health office.
- Life Threatening Allergy Care Plan
- Medical Statement to Request Special Meals and/or Accomodations (FOOD ALLERGIES ONLY)
If your child has seizures, please have your your Physician complete the Seizure Action Plan and return it to the health office.
**If your child needs to be out of PE for more than 3 days, they will need a doctors note.
